6. Some Dual Enrollment Plans (i.e., Medicare and Medicaid* combined by UHC)
*We can provide referrals for Medicaid plans, but your office visit would be self pay.
7. Blue Cross Blue Shield Plans (except MyBlue and BlueSelect)
8. Commercial Plans*
*We do not accept commercial and some marketplace insurance plans from UHC, BayCare and Aetna, except for health plans that cover out-of-network services.
The only HMO plan we accept from Medicare is Ultimate. We also accept Medicare Advantage PPO plans if they offer out-of-network benefits (except CarePlus, Devoted, Humana Gold, and Optum). Some PPO plans may also require copayment
Our Insurance Processes
Health plans vary from policy to policy. While we’ll help you process your insurance, we encourage you to check the healthcare services covered by your plan. Please contact your provider for the most up-to-date information regarding your insurance coverage.
Patients must present their insurance card at check-in. Any amount not covered by your health plans will be directly charged for self-payment. Copayment should also be provided at the time of services.
